We look forward to your upcoming appointment with Washington Urology.
In order to make your appointment as productive, efficient and as informative as possible you will need to Register Online.
Please arrive 20 minutes PRIOR to your appointment time.
Additionally you will need to bring the following:
Referral (if necessary)
Photo identification
Insurance information (primary and secondary)
Co-pay for a specialist visit (Visa, American Express, MasterCard, cash and checks accepted)
Pharmacy address, telephone number and fax number

List of current medications (prescription, over-the-counter, or herbal preparations) with dosage and frequency
Medical Documents:
Imaging: Reports and Actual Images (CD or film) of any previous imaging (ie: CT Scan, MRI, Renal Scans, Ultrasounds). If imaging done at Virginia Hospital Center you do NOT need to bring this with you
Laboratory Reports—bring any pertinent lab results
Summary or medical records from Referring physician, if applicable

Be prepared to update any changes in your personal information, insurance or medical history obtained at your original visit.
Please arrive 15 minutes before your scheduled appointment so that this information can be updated in your electronic medical record.

The physicians at Washington Urology provide coverage
24 hours/day, 7 days/week, 365 days /year.
If, when the office is closed, you have an urgent need that requires medical attention, please call the office main phone line 703.258.2838 and your call will be automatically routed to the answering service who will page the on-call urologist for you.
If you are experiencing a true medical emergency, call 911 for emergency assistance or go immediately to the Virginia Hospital Center Emergency Department or to a medical facility nearest your location.
All non-urgent calls including prescription requests and refills should be delayed until the office is open during normal business hours.

For routine medication refills, please call the Washington Urology prescription line at 703.717.4214. Be prepared to leave a voice message including your name, date of birth, phone number, medication name/strength/quantity, and your pharmacy phone number.
Allow one full business day for the prescription to be called in to your pharmacy. Any calls placed after hours or on the weekend will be addressed during the next business day.

At Washington Urology, we understand that you may need to change your appointment date or time.
We require 24 hour notification of any changes to your appointment. Changes made less than 24 hours prior may incur a charge.
If the doctor is delayed due to an emergency, we will make every effort to notify you prior to your appointment.

New patients to Washington Urology are encouraged to register online at the website prior to your appointment. Doing so will decrease your wait and expedite your visit with our clinical staff allowing your Provider to focus on your urological evaluation.
