First
Office Visit
We look forward to your upcoming appointment with
Washington Urology.
In order to make your appointment as productive and as
informative as possible, please be sure to bring along the following
information:
- Referral (if necessary)
- Photo identification
- Insurance information (primary and secondary)
- Co-pay for a specialist visit (visa, mastercard, cash and checks accepted)
- Pharmacy telephone
number and fax number
- List
of current medications (prescription, over-the-counter, or herbal
preparations) with dosage and frequency
- Completed New Patient Registration Form, which can be downloaded and filled out prior to your appointment.
- Reports and actual
images (CD or film) of x-rays, CT scans, nuclear medicine studies, or MRI
scans. If you have had your
imaging performed at the Virginia Hospital Center, we are able to access
your imaging through our computer system and therefore you do not need to
bring any films to your appointment.
- Reports of laboratory
studies
- Summaries or medical
records from referring physician
- If during the submission of your online registration you did not receive confirmation that your registration was accepted, please contact the office before your appointment.
Please arrive 20 minutes before your scheduled appointment so that this
information can be transferred into your electronic medical record.
Follow-up
Office Visit
Patients
who are coming for a follow-up visit will be asked to inform us about any changes
in the information or medical history obtained at the original visit.
Please
arrive 15 minutes before your scheduled appointment so that this
information can be updated in your electronic medical record.

After
Business Hours and Weekend Coverage
Washington Urology provides coverage 24 hours a day, seven
days a week.Should you have a
question after business hours or during the weekend that can not wait
until the next business day, please
call 703.717.4200 and you will be contacted by the on-call physician.In case of an emergency or if
your situation is critical, call an ambulance or go immediately to the Virginia Hospital Center Emergency Department
or to a medical facility nearest your location.

Prescription Refills
We request that
you call us for routine medication prescription refills during business hours. Prescription refills will not be filled
after hours or on weekends.
Insurance Plans
We participate with the insurance plans listed below.If you do not see your insurance
carrier included on the list, feel free to contact Washington Urology to
confirm that your plan is not accepted.
We
request payment at the time of your appointment for services that are not
covered by insurance. Your
insurance company may also require you to pay a co-payment at the time of your
appointment; you are responsible for payment of the co-pay before any services
are rendered. We accept Visa and
MasterCard.
Questions
concerning billing, accepted insurances or professional fees should be
addressed to our Office Manager during office hours.
Questions
regarding your plan's coverage and benefits should be directed to your employer
or insurance company.

Office Hours
Our regular
office hours are Monday through Thursday, from 8:30 am - 4:30 pm Friday from 8:30am - 3:00 pm. Please note that the office does
close each day for lunch from 12:00 pm – 1:00 pm.
If the doctor
is delayed due to an emergency, we will make every effort to notify you prior
to your appointment.
